How can you refer a patient to Connect Hearing?

1. If your patient is a pensioner:

To receive subsidised hearing care from the OHS, pensioners need a medical certificate to be authorized by their GP. Download our editable medical certificate:

 Once they have this signed certificate the patient is then able to book a free comprehensive hearing assessment with Connect Hearing. Download our editable referral form.

2. If your patient is employed, or a self funded retiree:
